 Police Checks are required for APMHA Coaches, managers and TrainersPolice checks will be done for Volunteer Coaches and Coaching Staff.
As of the 2010-2011 season, each individual requiring a Police Check will need to present themselves to the Carleton Place OPP detachment during normal office hours (815am and 430pm, Mon to Fri). You will need the completed Police Check form, 2 pieces of ID, one must be Photo (Health Card can not be one of the pieces of ID) and a Police Check Request form letter completed by the Director of Discipline, Risk and Safety - Kim Julian.
You will be notified by the OPP when your Police Check has been processed. Please pick it up as soon as possible and present it to the Director of Discipline, Risk and Safety - Kim Julian. A scanned copy of the processed Police Check emailed to the Director of Discipline, Risk and Safety - Kim Julian (dir.discipline.risk@apmha.org) is also acceptable.
The police check forms will be kept on file for the duration of the hockey season and sealed in a locked cabinet.
Volunteers may refer to the APMHA Privacy Policy - and can be assured all information is confidential.
Parent volunteers (other than coaching staff) who wish to have their police check done must follow the same process as outlined above. Generally speaking, if your participation requires you to have completed Speak-Out you will be required to have a Police Check completed.

Deadline for police checks is October 31, 2012. Volunteers who have not completed these checks by this deadline, NOT TO BE EXTENDED will not be permitted on the ice or bench as volunteer.
