REGISTRATION INFORMATION  - 2010/2011 Hockey Season

Welcome to another season of hockey in Almonte/Pakenham. 
The on-line Registration is now available.

YOU WILL NOT RECEIVE A REGISTRATION PACKAGE BY MAIL

 

Our On-Line registration is open - early bird fees will apply until June 30th 2010.

 

The on-line registration offers payment via credit card or cheque. The on-line registration enables us to process the registration faster and more effectively and we appreciate your cooperation.

 

The following documents will be distributed and collected by each team manager at the beginning of the hockey season when the teams are formalized: Parents and Player Fair Play Pledge, Publication of Information Form.

 

 This year:

·         Unlike most associations in the ODMHA, APMHA has not increased the fees for the 2010-2011 hockey season!

·         Option with installment payment (cheque or Credit Card): 3 installmentsà June 15th  or at the time you are completing your on-line registration; August 1st and September 1st.

·         On-line registration for Competitive players and players who plan to attend competitive tryouts (MTK, OVSS and OVT) – payment by cheque or by credit card with installment with APMHA. Fees will be then transferred by APMHA to the Competitive association upon successful placement in a competitive team. You will be then asked to proceed with the additional payment required by the competitive association. All competitive associations require that registration and payment must be completed with the home association before the first competitive tryouts.

·         The mandatory $80.00 raffle ticket price is added to your registration fees this year. Raffle tickets will be distributed by your team manager once the teams are formed. You can sell you raffle ticket or keep it for yourself. The draw will be before Christmas 2010. For competitive players the $80 raffle ticket equivalent will be transferred to the competitive association as part of your fees collected by APMHA.

·         An additional and mandatory $60 has been added to all midget players’ fees (jersey caution) that will be reimbursed to parents at the end of the 2010-2011 hockey season upon the return of the player’s jerseys. We are starting to replace our Game jerseys this year and we are starting with the midget teams.

Please note: The third player in a family may register for $100. The third player is considered the player with the lowest registration fee.
¹ - We are considering adding 4 years old players this season. To participate in our Initiation program, a 4 years old player must be able to skate unassisted.
-  Age 7 players have the option of playing Novice or remain in the IP program.

* Players who are of ages 7 and 8 will be eligible to play Novice hockey. However, children who are 7 years of age can remain in the Initiation Program should the parent(s) feel that their child requires an additional year of development as provided through the Initiation Program. APMHA will conduct our normal player evaluations during which we will pay special attention to the 7 year olds to ensure the players are placed in the correct program based on their skill level.

ANY REGISTRATIONS RECEIVED AFTER JULY 1st  WILL BE PLACED ON A WAITING LIST AND WILL BE HANDLED ON A FIRST COME FIRST SERVED BASIS. Processing of a registration on a waiting list will occur when and if there is space available for the player.

On-Line registration with Payment by cheque

For registration with payment by cheque: please mail the ON-LINE registration confirmation with payment to the registrar address before June 15th, 2010. All cheques must be made payable to APMHA. When registering on-line you can select to make a cheque with full payment (cheque post-dated to August 1st  2010) OR you can select to pay the fees with 3 post-dated cheques - June 15th , August 1st and September 1st 2010 ;

NSF and Refunds

There will be $25.00 charge for all NSF cheques received and they must be replaced within 7 days of notification by cash, certified cheque or money order. Failure to do so will result in the risk of the player being put on a waiting list and the member declared "not in good standing" with the association.

The registrar must receive refund requests in writing by December 15th and no refunds will be given for any reason after December 31st.Refund formula is as follows: (cost of registration - $25.00) x [(# of FULL months left in season) / 7]. A full refund less a $25.00 administration fee will be given to anyone withdrawing before September 1st.

ADDITIONAL FEES: The APMHA Executive sets the APMHA registration fee annually. Total cost per player above the APMHA registration is estimated at $150.00 (Novice to Midget). This additional cost would include tournament fees, exhibition games, etc.

Players registering for the first time with APMHA

Players who have never been registered with any hockey association prior to this year, please mail a copy of the following documents with your on-line registration confirmation:

- A copy of a valid birth certificate
- A copy of a valid proof or residency (driver's license, Electricity or phone bill)

Players who were registered with another hockey association last year, please mail a copy of the following documents with your on-line registration confirmation:

- A copy of a valid birth certificate, 
- A Declaration of Residency (DOR available for download in the "additional forms" section of this page). New residency must have an effective date no later than July 31st 2010.
- ODMHA Transfer of Application Form
(available for download in the "additional forms" section of this page).


Conditioning Camp

Conditioning camp will be offered again this year. The costs associated with the conditioning camp are waived for the upcoming season as a result of APMHA Golf Tournament. The camp is available to all Novice age players and above. You will be contacted at the beginning of the season by the appropriate coordinator for the scheduled times of the camp.

With every hockey season our association requires the support of officials so our kids can play the game that we all love.
