Please visit our website www.apmha.org, choose "for parents" and "account info". Here you will enter your player's name, birthdate and access code. You will see the option for tax receipts - choose the tax year from the drop down and you will be given the option to print the reciept or have it emailed to you.
If you do not know your access code please follow the link to obtain it (access code will be emailed to the email you entered when you registered your player)
