Registration for the 2014-2015 Season 


There is a delay this year in registration, as we are waiting for the goalline website to complete an update.   As soon as the update is completed we will open early bird registration.  The Early Bird rate will be extended beacsue of this delay.  In the interim you can find all pertinant info below, including in person registration dates and fees.

In Person Registration
Sessions:
Thursday June 5th 7:00pm – 9:00 pm at the Almonte Arena
Thursday June 26th 7:00pm – 9:00pm at the Almonte Arena.
 
Make sure you include the following in your package:
1. Completed registration form 
2. Please make cheques made payable to APMHA installments can be made in 3 equal payments dated on the date of registration,  August 1, 2014 and September 1, 2014)
3. Copy of Birth Certificate (new players only)
4. Copy of one of the following documents to prove proof of residency (new players only): utility bill, tax bill, lease/rent agreement, bill of sale of home  ****Please note that a copy of your telephone bill is NOT acceptable.

NOTE: * Registration amounts cover the increase of ice rental (3%/$20 per player as well as the increase in HEO fees of $12 per player) 
 
Additional Registration Information

  • For parents of children with 3 or more players, the fees for the youngest child will be $100 
     
  • A $25 charge for all NSF cheques received and must be replaced within 7 days of notification by cash, certified cheque or money order. Failure to do so will result in the risk of player being put on a waiting list.
  • Registrar must receive refund requests in writing by December 15th and no refunds will be given after December 31st. Refunds are pro-rated depending on the number of months attended.
  • Any player transferring to a competitive team or neighbouring association must have a completed registration with APMHA prior to the transfer taking place.
  • Full transfer minus $25 processing fee will be applied to any player transferring to a competitive team or a neighbouring association
  • Full refund minus $25 processing fee will be given to any player withdrawing before September 15th.
